Request for Transportation Form

Based on the Procedure 200.4.02 - Transportation of Students in Buses and Other Vehicles:
E. Guidelines
1.       General Safety
         a.       Transportation of Students to and from school must be in Type I, II, or III School Buses.
         b.       Transportation of Students in Type I, II, or III School Buses, or a Motor Coach shall be limited to school-related functions and activities only.
         c.       Employees shall follow the exemption process outlined below Criteria for Exemption for Bus Transportation.

Criteria for Exemption for Bus Transportation
  1. Requests for exemption to use a non school bus for each trip/activity shall be submitted through the KS Transportation Request Form and may only be granted by the Po'o Kumu or designated school administrator if the request meets at least one of the following criteria:
    • A School Bus is unavailable during the specific time requested to meet the needs of the activity.
    • The nature of the activity requires a specialized vehicle to reach a difficult destination (e.g. 4 wheel drive).
    • The length and time of the activity require the use of the bus beyond the regular school day.  For example, the activity requires the use of the vehicle before, during and after school and/or evening hours.
  2. KS Drivers who transport Students in a KS Passenger Minivan/Vehicle must submit a KS Request for Transportation Form.
  3. Document Retention - Copies of all approved exemptions shall be maintained by the Transportation Manager at each educational site.
